Editor’s note: So here comes news that it isn’t the insurance industry that’s the driver, but providers. Maybe we’ll all come to our senses and realize that all stakeholders are responsible for the out of sight costs — including the inelastic demand for health care by patients, when they are only directly paying a small portion of the total cost.
